It is mandatory with the state of Utah for individuals that substitute or drive for school sponsored events to

complete a fingerprint and background check.

 Paradigm High School will not reimburse you for these fees.

You will receive 6 volunteer hours.

Instructions for Fingerprint & Background Check:

1. Fill each form out completely. We cannot accept any paperwork until this is done.
2. Give the Waiver Statement to Paradigm High School with $25 check or cash (check made out to Paradigm High School).
3. You will then need a Paradigm High School official’s signature on the Request for Electronic Applicant Background Check. After that has been done, you will take that form with $25 to any Utah Educator Live Scan Fingerprint Site. While you may go to any site listed on the following page, we recommend Jordan School District. They take CASH ONLY
