Attached are some important forms that you might need at various times.

Scheduled Absence Request: Parents may schedule a student’s absence for a period of up to ten school days by filling out the Scheduled Absence Request form and submitting it at least two days before the absence occurs. If the absence is approved by the school, these days will not be included in the absences affecting class credit, or the 10 absences affecting school enrollment. The student is responsible for making arrangements with the mentors and completing the work for his/her classes during this time.

Shadow Request: Students may have a friend or relative of middle school or high school age attend classes with them on campus (“shadow”). The purpose of this is to introduce prospective students to Paradigm prior to enrolling and is not allowed for previous students. Permission forms for shadow visits need to be turned in the day of the visit to the front office. No student may bring a guest without permission from the administration. Student guests must attend in full dress code, follow all school rules, and wear a “Visitor” badge while on campus. No student guests will be allowed the final two weeks of either semester.

Field Trip Permission Slip:  When students attend any field trip they need to have parent permission.

Student Parking Permit: Any student that drives themselves to school needs to have a parking permit. Please fill out the attached form and turn it in to the office with $5.

Home School Portfolio: This is a guideline for how to write up home school credit for students that have been home schooled during high school.

Off Campus Study Agreement: This is for students with any form of release time in their schedule.  Please fill out and return to the front office.
